RESUMO

Introduction@#Hepatocellular carcinoma incidence and mortality per 100,000 population in Mongolia is the highest in the world. The individual’s genetic factors and new genetic changes are considered an important effect on the origin and development cancer. We aimed to investigate whether p53R72P polymorphisms were associated with the risk of hepatocellular carcinoma in Mongolian patients.@*Material and Method@#p53R72P polymorphisms were evaluated in 80 controls and 38 HCC cases using a PCRrestriction fragment length polymorphism assay.@*Results@#The mean age was 58.5±13.6 years in the case group and 63.2±8.1 years in the control group. Hepatocellular carcinoma is most common in 50-59 (n=14, 36.8%) and 60-69 (n=14, 36.8%) ages. Of the HCC group, 4 (10.8%) were diagnosed with tumor at stage II, 23 (62.2%) at stage III, and 11 (27%) at stage IV. </br>The results revealed that the heterozygous (Arg/Pro (PR)) genotype of p53R72P increased statistically significant the risk of hepatocellular carcinoma (OR=4.222, 95% CI 1.669-10.684) compared to the wildtype (R/R) genotype. (p=0.002). Moreover, the homozygous (Pro/Pro (P/P)) genotype of p53R72P increased the risk of carcinoma (OR=1.333, 95% CI 0.414-4.299) but not statistically significant. (p=0.63). Heterozygous (Arg/Pro (PR)) genotype of p53R72P in the tumor tissue was associated with a statistically significant (OR=3.3, 95% CI 1.274-8.57) increase in the risk of HCC (p=0.014). Pro/Pro (PP) genotype increased the risk of the carcinoma by 2.4 times (OR=2.44, 95% CI 0.865-6.908), but it was not significant. (p=0.092). Pro/Pro (PP) genotype of p53R72P in the tumor tissue compared to normal tissue of a case group increased the risk of cancer by 1.8 times (OR=1.833, 95% CI 0.472- 7.126), which was not statistically significant (p=0.382).@*Conclusion@#Taken together, Heterozygous (Arg/Pro (PR)) genotype of p53R72P increases the risk of hepatocellular carcinoma in Mongolians. Further studies with larger populations are needed to confirm these results.

RESUMO

Background@#Infectious keratitis is a disease caused by inflammation, infection, and other ocular damage to the outer and other deep layers of corneal epithelium. It is a major cause of monocular blindness and visual disability worldwide regardless of age and gender. Therefore, we aimed to determine the clinical features and risk factors of infectious keratitis among Mongolians and to identify the causative microorganism and compared them with the treatment results.@*Methods@#We collected the data of 149 patients who diagnosed as infectious keratitis at the Ophthalmology Department of the First State Central Hospital in 2017-2020 and using a case series model of descriptive study. Statistical analysis was calculated using Stata14 software.@*Results@#The majority of patients in our study were male, with a male-to-female ratio is 2.1:1. The cause of infectious keratitis were categorized and eye injury-induced keratitis accounted for the highest percentage of 38.3% (n=57), with the majority being men 73.7% (n=42) (p=0.028). As for the type of treatment, antibacterial drugs 103 (69.1%) and evisceration 27(45%) predominate.@*Conclusion@#Our study shows that the majority of infectious keratitis in Mongolia is due to trauma in male patients. Forty-five percent of all surgeries involve evisceration surgery, which reduces the client's quality of life.

RESUMO

Background@#Lung cancer is the leading cause of cancer death in both men and women. In the past decade, histopathological classification of lung cancer has become increasingly important in selecting the appropriate treatment.@*Methods@#All cases of lung cancer diagnosed pathologically in the last 2.5-year period were retrospectively identified at National Cancer Center and performed by descriptive case series design.@*Results@#The average age of our participants was 62, of which 82.2% were men. A total of 214 cases of lung cancer were reported, including 142 (66.4%) squamous cell carcinoma (SCC), 34 (16%) adenocarcinoma (AC), 4 (1.7%) NSCLC, 29 (13.6%) small cell carcinoma (SmCC) and 5 (2.3%) cases of other/unspecified type. Based on only morphology, initially 32 (17.7%) cases of NSCLC were diagnosed, whereas after immunohistochemistry (TTF-1 or NAPA positive/p63 negative, and vice versa) staining, this category reduced to 2.2%.@*Conclusions@#Among both males and females, SCC which is highly related to smoking was the most common histologic type of lung cancer. Our results showed that an approach of using only a two-antibody panel (p63 and TTF-1) might help in the reduction of diagnostic category of NSCLC-NOS significantly.

RESUMO

@#BACKGROUND: According to WHO, Non-communicable disease take place 43% of morbidity as well as 63% of mortality in worldwide and it will may increased as 60% of morbidity, 73% of mortality in 2020. Therefore one quarter of NCD associated with composition and usage of foods. Obesity and Diabetes mellitus are caused by accumulating saturated fat in body due to excessive use of high calorie. High-fat diet promotes lipid deposition in solid organs including skeletal muscle which is underlying cause of insulin resistance. METHODS: Same aged, Inbred (C57BL/6J) 20 male mice are involved in this study. They are raised and fed in central core laboratory of MNUMS. Feeding of mice “Beijing Branch of the Joint Feed Co LTD” from Republic of China produces forage for laboratory animals. They were divided into two groups. A control group mice had been fed standard chow while experimental group mice had been fed 45% high fat diet for 4 weeks. The morphological effect of the a high fat diet was examined by light microscopy. RESULTS: Liver of experimental mice was seen pale and parenchyma had a fatty changes compared to control group liver in gross anatomy. Periportal necrosis and lipid droplet deposition in hepatocytes of study group were detected as microscopically. There is fatty accumulation between solid tissue of pancreas and islets of Langerhans get smaller in study group. Experimental mice which were fed by 45% of fat containing food had lipid droplets were observed in fibers. Therefore degenerated fibers showing separation from the basement membrane and acidophilic staining were observed in skeletal muscle.

RESUMO

@#BACKGROUND: In the recent years, mesenchymal stem cells have become increasingly utilized in regenerative medicine and tissue engineering applications because of their properties for self-renewal, differentiation and immunoregulation. The use of stem cells of various clinical applications is highly expected and the production of good quality stem cells is very critical for basic studies. In the bone marrow, hematopoietic and mesenchymal stem cells from an unique niche in which the oxygen tension is low. Hypoxia may have a role in maintaining stem cell fate, self renewal and multi-potency. We investigated whether low oxygen culture would be beneficial for hematopoietic stem cell and mesenchymalstemcell. MATERIAL: BMCs from 8-12 week aged, 15 mice were subjected to hypoxic conditioning by culture for 8-10 days in 20%, 3%, 1% oxygen. For culture 1x105cell/ml were seeded in colony forming assay and 2x106cell/ml were seeded in L-glutamin mediain chamber slide. We counted cell colonies under different hypoxic condiontins by Olympus IX71 fluorescence microscope. After cell culture in chamber slide, we stained cells by anti-CD90 and anti-CD105 then counted positive cells by Olympus IX71 fluorescence microscope. RESULTS: Compared to normoxic cells and hypoxic cells well morphologically differentiated and counted by Olympus IX71 microscope. More colonies were observed at 3%, 1% oxygen. Statistical significances were identified with granulocytes and macrophage colony (p<0.05) in hypoxic condition. More anti-CD90 and anti-CD105 markers were observed at 3% oxygen condition. Statistical significances were identified in 3% oxygen condition with cell markers(p<0.001). CONCLUSIONS: Our data suggests low physiological oxygen culture could improve the stemness of macrophage and granulocytes colony and improve the differentiation of mesenchymal cells. Long term culturewith additional cell markers will be necessary to confirm whether low physiological oxygen levels also improve genomic stability

RESUMO

Introduction Cervical cancer screening program is using conventional Pap smear (CPS) have been successfully used in Mongolia, but new kind of test as “Liquid based cytology” (LBC) is not popular for screening. This liquid based cytology testing might reduce the number of unsatisfactory smears, and increase the accuracy of diagnosing. Goal The main aim of this study was to assess diagnostic accuracy and sensitivity of Liquid based cytology versus Conventional Pap smear using ‘split-samples’ technique. Materials and Methods This was cross sectional study, total of 75 cervical split samples were included over a period of 2 months. Split sample was obtained using cervix-brush. CPS was prepared from brush and the brush head was suspended in the Liquid based vial and processed by LBC method and pap staining. Smears were diagnosed by cytologists. Abnormal smears were concluded by cervical biopsy as a Golden Standard. Results There were 14%unsatisfactory smears in CPS and 6% in LBC; the main cause is insuffi cient cells, and excess blood in CPS. LBC had diagnostic accuracy of LSIL was lower, but ASC-US was higher than CPS was signifi cant. LBC (100%) was more sensitive than CPS (89%) was confi rmed by biopsy. Conclusions: LBC testing was successfully reduced unsatisfactory smears rate. LBC samples offered better clarity and uniform spread of smears, less time for screening and better diagnostic accuracy of LSIL and ASC-US than CPS.

RESUMO

In United States, person is diagnosed with blood cancer in every 3 minutes. In 2015, there were 1.665.540 total cancer cases and 9.4% of them lymphoma and leukemia . In 2015, there were 95 cases of lymphoma in Mongolia when compared 4 times increase to 2015 from 2010 . Bone marrow examination is an established diagnostic modality in the evaluation of various hematological disorders. BM examination can serve to establish or confirm a primary diagnosis of lymphoma or to determine the extent of disease dissemination for staging purposes. Biopsy is essential for diagnosis in a dry tap or blood tap which occurs when the marrow is fibrotic or densely cellular. Only a biopsy allows a complete assessment of marrow architecture and pattern of distribution of any abnormal infiltrates. In 2015.01-2016.01 fifty five bone marrow biopsies were retrieved from the files of the National First Clinical Hospital-Department of Hematology. These all statistical analysis was performed using by SPSS 17. Bone marrow processing and staining: The hematologist is instructed to place the freshly obtained BMTB specimens directly into buffer substance fixative and transport it immediately to the histopathology department, on the same day as the procedure. The next morning (after 20–24 h), the solution is decanted (with a strainer) and the biopsy specimen is washed in distilled water for 30 min. The biopsy specimens are left to decalcify for about 6 h before being processed and embedded in paraffin wax, with procedures similar as for other specimens.Sections, 1-mm thick (microtome set for 1 mm sections), are cut from the paraffin-wax blocks with the routine rotary microtomes in the laboratoryA total of 55 cases were reviewed from December 2014 to November 2015. The age of the subjects ranged from twenty two years to seventy eigth years with a male predominance (1.7:1). Data of 55 trephine biopsies were reviewed. The percentage of trephine biopsies in different length ranges was calculated. Twenty two biopsieswere of recommended length, i.e., ≥1.5 Cm while remaining 33 were less than the recommended length. The rate of positivity for diagnosis was 95.4% in group-1, 94.1% in group-2, 63.6% in group-3 and 40% in group-4 In all cases in our study 73% (n=40) were satisfactory and 27%(n=15) unsatisfactory slides .Our study showed that 40% trephan biopsies were of revommended length i.e >=1.5cm with 95.4% positive of diagnosis. However biopsies measuring 1-1.4 cm also had comparable result 94.1% .

RESUMO

Thyroid nodular lesions are the common clinical problem in the world. A variety of tests have been employed to separate benign from malignant thyroid nodules. These tests include isotope scanning and fine needle aspiration cytology (FNAC). Our research was based on the fact that the comparison of FNAC and thyroid isotope scan in thyroid nodule was not researched in Mongolia. Therefore, we want to evaluate the specimen adequacy of FNAC, and compare its result to thyroid isotope scanning in patients with thyroid nodule. The research was conducted on archive materials of FNAC and thyroid isotope scanning of patients with thyroid nodule who were treated in surgical department of The First Central Hospital from 2012 to 2015, and the statistic analyze was done by using SPSS 20 under the auspices of School of Pharmacy and Bio-Medicine,Department of pathology, Mongolian National University of Medical Sciences. We have collected the FNAC of 807 patients, including 34 patients (4.2%) were males and 773 patients (95.8%) were females. FNA cytology results were interpreted as benign in 495 cases (61.3%), follicular lesion of undetermined significance in 31 cases (3.8%), follicular neoplasm in 9 cases (1.1%) suspicious in 17 cases (2.1%), malignant in 3 cases (0.4%), and unsatisfactory in 252 cases (31%). We have got the result of thyroid isotope scanning of 43 (5.32%) patients out of 807 cases. On thyroid scan, 18 patients (41.9%) having cold nodule were labeled as suspicious for malignancy, 25 patients (58.1%) had hot nodule. The FNA diagnosis of 25 patients with a hot nodule following: 1 patient (4%) with neoplasm, 17 patients (68%) with benign results, 6 patients (24%) had non-diagnostic. The FNA diagnosis of 18 patients (41.9%) with cold nodule following: 1 patient (5.5%) with follicular lesion of undetermined significance, 10 patient (55.5%) with benign, 5 patient (27.7%) with non-diagnostic, 2 patient (11.1%) had malignant. The FNAC results were interpreted as benign in 495 cases (61.3%), malignant in 3 cases (0.4%), and unsatisfactory in 252 cases (31%). Two patients out of 18 patients with cold nodule diagnosed as malignancy by FNAC. The sensitivity and specificity of thyroid isotope scanning was 96% and 16.6%, respectively.

RESUMO

IntroductionStudies on structure and function of reproductive system of experimental animal by developing coldstress model have been getting attractions from researchers. The study on changes of reproductive system of offspring from experimental animal which had cold stress has been done in recent times.GoalCold stress modeling in female experiment rats and, identifying the changes of stages of estrous cycleand reproductive hormone.Materials and MethodsIn order to identify stages of changes of reproductive hormone of experiment female rat which is model of cold stress, 90 Wistar breeding rats weight of 170-178 g were divided into following 6 groups:Non-gravid control group–15, gravid control group–15, non-gravid group for cold stress–15:15:15, gravid group for cold stress–15. Pathologic cold stress was modeled in 15:15:15 rats of non-gravid group for cold stress by putting in fridge everyday between 8 to 11 o’clock for 7:14:21 days and in 15 rats of gravid group for cold stress by putting in fridge (-150) every day between 8 to 11 o’clock for 28 to 30 days (Avcina AP(1989), Melovanov АP(1991)). Time to vaginal opening was determined to use method of Volkova OB in offspring of gravid experiment rats and control group after 2 months. After that selected 15 female offspring were put to sleep with ether and taken blood of 4-5 ml from their hearts and compared the amount of estradiol and progesterone in that blood using rat kits of Ausmausco Pharma with control group.Result Determining hormone level in blood of control group, estradiol is 4.73±0.45ng/l, progesterone is 2, 45±0,50ng/l. After a week pathologic cold stress was modeled in rats, estradiol is determined 3.93±0.70ng/l, progesterone is determined and after 2 weeks, estradiol is 4.0±0.53ng/l, progesterone is 1.97ng/l and after 3 weeks, estradiol is 3,8±0,63ng/l, progesterone is 1,94±0,09ng/l Birth weight of offspring from gravid rats with cold stress, was 1,6g (р=0,001) less than control group.Time to vaginal opening of offspring was 14 days later than control group. Determining hormone of control group offspring, estradiol is 4.53±0.51ng/l, progesterone is 2,39±0,40ng/l. Estradiol is 3.73±0,59.ng/l, progesterone is determined 1.97±0.07ng/l in offspring of gravid experiment rats. Conclusion: 1. When develop cold stress model, gravid rats had miscarriage and preterm delivery and born low birth-weight offspring in 16-22 days of experiment and this reveals that pathologic changes occurred in sexual hormonal regulation of 3 groups of non-gravid rats of experiment because of cold stress. 3. In offspring from gravid rats which had cold stress time of vaginal opening was lengthened and estradiol and progesterone level was decreased in offspring from gravid rats and

RESUMO

Today, in technologically updated world medicine, laparoscopy is more preferable than the common, open surgery procedures. There is a lack of scientific studies on laparoscopic surgery in gynecology, especially its present state and further tendencies. Therefore it served us as the background of the study.Retrospective study was conducted on 2016 laparoscopic surgery histories, which were performed in 2010-2014, at Mongolian state and private obstetric-gynecological hospitals and clinics. The study was developed by the program SPSS21.In 2010-2014 totally 2016 laparoscopic surgeries was performed: 713 – in First Maternity Hospital, 653 – in Bayangol hospital, 429 - inASE hospital, 221 – in National Cancer Center of Mongolia. The patients were classified by their age, education state, and citizenship. Mean age of these patients was 33.03 ±7.6(min 12, max 70). By educations: the 72.4% (1460) of them were high educated, 18.2%(367) has the secondary education, 4.7%(95) – college education, 4.5%(91) – 8th grade education, and 0.2%(3) – elementary school education or non-educated. The 81.8 %( 1649) patients were citizens, the 18.15 %( 366) – from rural areas. And only 0.05% (1) was the foreigner. We noted 7 %( 14) cases that shifted from laparoscopic surgery to open procedure. The post surgery average bed day was 2.7 ±1.3 (min 1, max 12), duration of the laparoscopic surgery 58.46 ±31.6 minutes (min 10, max 260). The average amount of blood loses during the surgery was 76.17 ±69.3( min 5, max 700),The relevance of the surgery duration and preoperative bleeding was(r-0.445). Various diagnoses were involved in the study: the 25.9% (524) of them had second infertility, ovarian endometriosis cyst- 10.5% (212), ectopic pregnancy- 9.4% (189), hydrosalpinx- 7.5% (152), endometrioma ≥ 4.5 cm - 6.3% (128), ovarian follicularcyst- 6.3% (128), ovarian cancer- 5.7% (115), primary infertility- 5% (101), ovarian dermoids cyst-4.3% (86), endometrioma ≤ 4.5cm- 3.4% (68), others-15.7%. Surgical procedures are classified by their purposes: 29% (584) of surgery for examine the fallopian tube using contrast for medium and divide adhesion, 17.6% (354) – to remove ovarian cyst, 12.8% (259) – to examine fallopiantube using contrast and remove ovarian cyst, 9.4% (189) – to remove fallopian tube, 7% (142)- to remove endometrioma, 6.3% (128) – to examine uterine tube using contrast dye, 6% (121)-ot do hysterectomy, 11.9%- for other reasons. The 76.7% of all surgeries were done within one hour and there was moderate correlation between duration of surgery and bleeding (r-0.445). The secondary infertility was the most common diagnosis. And the laparoscopic manage of adhesion and hystersalpingography were performed more. According to the study, non-cancerous growth of uterus was 9.7 and the 6% of it surgeries was laparoscopic myomectomy.

RESUMO

Background:Laboratory test are done on clinical specimens in order to get information about the health of a patient as pertaining to the diagnosis, treatment and prevention of disease. Laboratory test gives 70% of information to get a right diagnosis. By some study, physiological factors such as diet, stress, exercise and sample collection techniques are influencing 32%-75% of test accuracy and reliability. That is why we want to studyhow nurses follow standard of sample collection techniques in UB.Our study conducted in 5 different hospitals. We observed nursesway of collecting sample from 150 patientsResult:Nurses did not identify patients ID in 80% of patients and did not ask test preparation and diet of 100%. They prepare necessary items to blood draw 95%. Also nurses did not fully follow blood draw standard in such way: hand sterilization, asepticize place of puncture and using bandage. Conclusion:Nurses don’t follow standard of draw blood from vein and did not clarifies patients test preparation. Pre-analysis process is the most influencing factor in the test result. So we have to train nurses to follow their standard of sample collection procedure.

RESUMO

Background:Mesenchymal stem cells derived from bone marrow and adipose tissue are being applied to tissue engineering and cell therapy. The use of stem cells of various clinical applications is highly expected and the production of good quality stem cells is very critical for basic studies. In the bone marrowmesenchymal stem cells from an unique niche in which the oxygen tension is low. Hypoxia may have a role in maintaining stem cell fate, self renewal and multi-potency. We investigated whether low oxygen culture would be beneficial for mesenchymal stem cell. Results:BMCs from 8-10 week aged, 6 mice were subjected to hypoxic conditioning by culture for 7 days in 20%, 3%, 1% oxygen. For culture 1x106 cell/ml were seeded in media with L-glutamine in each dish. During the culturing, cell colonies were checked once in three days. After cell culture, we stained cells by CD90 then counted CD90 positive cells by fluorescence microscope. More colonies and mesenchymal cells were observed at 3%, 1% oxygen and also colonies were bigger in hypoxic condition. Statistical significances were identified mesenchymal cells (p<0.05) in hypoxic condition. Conclusions:Our data suggests low physiological oxygen culture could improve the differentiation of mesenchymal cells. Long term culture will be necessary to confirm whether low physiological oxygen levels also improve genomic stability.
